High Blood Pressure and High Cholesterol Management
If you are dealing with high blood pressure, high cholesterol, or both, understanding your numbers and getting them to a safe, consistent level can significantly improve your health and reduce your risk for heart disease, stroke, and even dementia.
But it's not easy - and the fact that you may feel just fine in spite of your numbers doesn't help.
You can learn how to improve your numbers, and get encouragement and support, through the High Blood Pressure and Cholesterol Management program offered by Mission's Health Education Center.
The program includes both individual and group consultations with a team of experienced, specially trained educators. They include a nurse educator, pharmacist, dietitian, certified clinical social worker/behaviorist and an exercise specialist.
In this program you will receive an initial assessment to help us and you better understand where your blood pressure and cholesterol levels are. According to this assessment, we will help you set specific goals through three educational classes that meet once per week for two hours. In these classes, we will provide you with information on nutrition, your medications, stress management, exercise and activity, as well as general care guidelines for high blood pressure and cholesterol.
The goal of the program is to help you get your numbers to the healthiest level possible, so you can feel better, reduce your medications, or possibly be able to live medication free. Once your blood pressure and cholesterol numbers are at their target, you can continue meeting with your care manager, as much as once a month or a little as quarterly - whatever helps you maintain your healthy lifestyle. Our goal is the same as yours, to stay at your optimal health as a result of the positive lifestyle decisions you have made!
