My Healthy Life Wellness Program is voluntary and is designed to help participants reach health goals, stay motivated, and save money.
To qualify, one must be covered by Missions Health Insurance, and have one or more of the following diagnosis : Asthma, COPD, Diabetes, High Blood Pressure, High Cholesterol, and/or Depression.
Employer (Mission) will:
- Pay 100% of Diabetes and/or Hypertension/Cholesterol education classes
- Reduce or eliminate disease-related medicine co-pays and supply costs (blood glucose monitoring supplies)
- Diabetes and Asthma-100% waived co-pays on all medications
- Depression-100% waived co-pay on depression related name brand drugs when a generic isn't available. If the participant chooses a name brand drug when a generic is available, the cost will be the price difference between the generic and name brand drug.
- Hypertension, Cholesterol and COPD-100% waived co-pay on disease-related generic brand drugs--$5.00 co-pay on disease related brand name drugs.
- Pay 100% of initial education assessment.
- Pay 100% of required follow-up visits with the assigned care manager (every 1-3 months).
- Pay 100% of required lab work/breathing tests (every 6 months-1 year)
Participants part:
- Agree to complete education classes related to diagnosis within 90 days of enrollment. Topics include Nutrition, Activity, Stress Management, Medications, and more.
- Agree to meet with a specially trained Care Manager on a regular basis (every 1-3 months).
- Agree to have lab work/ breathing tests done every 6 months to a year (whichever is applicable).
Participants need to use Medical Center Pharmacy or Mission Community Pharmacy for the medications and monitoring supplies covered by the program. Insulin Pump users must use Liberty Diabetic Supply company for pump supplies-1-800-INSULIN (467-8546).
