To document that DaVita is providing hemodialysis patient care, treatment, and services in a safe and effective manner, we are requesting you complete this online survey. 

If you are a physician that your patients do not utilize hemodialysis you will answer "No" and submit your survey.  Only physicians who have patients that utilize hemodialysis will need to complete the 7 questions.  Please feel free to add your comments.  All survey need to be submitted by September 30, 2009.
