Out-patient Cardiac Rehabilitation

Out-patient Cardiac Rehabilitation

Once released from the hospital, patients may participate in Outpatient Heart Path with a referral from their doctor. The initial outpatient program usually lasts 12 weeks. Upon completion of the 12 weeks, patients may choose to attend the Heart Path Maintenance Program, join a local gym, or exercise at home.

Patients receive personal assessments to help develop their care plan, which can include:

  • exercise therapy
  • nutritional counseling
  • vocational and avocational counseling
  • psychosocial counseling addressing issues such as depression, anger, and quality of life


Mondays, Wednesdays, and Fridays are dedicated to exercise sessions, individual instruction, and group lectures. The formal lecture series includes topics such as:

  • physical activity
  • smoking cessation
  • anatomy
  • stress
  • medications
